Before you start your PAVE application, complete these essential preparation steps: 
 Define your service delivery scope by deciding:

  • ​​​​​​​What specific services you intend to provide (ECM, Community Health Worker, Doula, etc.)
  • Which populations will you serve
  • What geographic areas/counties will you cover 
 Map your planned services to:

​​​​​​​

  • ​​​​​​​Current revenue streams
  • Expected payment  mechanisms
  • Contract requirements with managed care plans 
Determine which Medi-Cal managed care plans or payers you will contract with for  each service type
 
NPI Types Explained:
NPI Type Who Needs It Description
Type 2 NPI Organizations For counties, non-profit corporations, and organizational entities. This is what CBOs must have.
Type 1 NPI Individuals For individual practitioners, sole proprietors, and licensed clinicians.This is what CHWs, Doulas, LCWSs, LPCCs, and LMFTs need.
 Note: licensed providers/doulas will need both a type 1 and type 2 NPI unless they are only operating as a sole proprietor. (The individual needs a type 1 and entity needs a type 2) 
 
HOW TO APPLY FOR AN NPI
  1. 1 Visit the National Plan and Provider Enumeration System (NPPES) website
  2. 2 Complete the online application (typically takes 20–30 minutes)
  3. 3 Receive your NPI in fewer than 10 business days. You can track your application status online
  1. 1 Download the CMS NPI Application/Update Form
  2. 2 Mail completed form
  3. 3 Processing takes approximately 20 business days
(THIS STEP IS FOR  NONPROFIT CBOs) 
 Before starting your PAVE application, collect these required documents:​​​​​​​

  • ​​​​​​​Federal Tax ID/Employer Identification Number (EIN)
  • Articles of Incorporation for your organization
  • Copy of lease agreement for your business location
  • Organizational NPI (Type 2 for organizations)
  • Individual practitioner NPIs (Type 1 for licensed clinicians)
  • Business License (even for solo practitioners)
  • Business address - CANNOT be a P.O. Box or residential address
  • Insurance information - Must match the business address
  • 501(c)(3) documentation (for CBOs)
  • Board Member and Officer Identifying Information (names, residental addres, birth date, SSN, see below.)
 
Know in advance: You will need to submit identifying information for your Board of Directors. You will need to submit their names, addresses, and social security numbers along wtih a statement of their relationship to your organization.

Important points:
  • Some Board members may be reluctant to share this information
  • This information is required - no exceptions
  • The information is securely transmitted and maintained by the state​​​​​​​
ACTION ITEM: Seek your board approval to move forward and provide this information before pursuing the Medi-Cal enrollment process.
