DOULA ENROLLMENT REQUIREMENTS
Any doula that formed a legal entity for their business (LLC, corporation, partnership) will use a National Provider Identifier (NPI) Type 2 to apply to PAVE. They will select either an individual or group practice.
CLARIFYING NPIs:
The NPI type is not dependent on whether the enrollment is for an individual or a group, but depends upon how the business is organized.
A sole proprietor business will always use a Type 1 NPI
An LLC or corporation will always use a Type 2 NPI (even it if is a single person.)
1 Obtain your NPI (type 1 for individuals and/or type 2 for LLCs and corporations) - for application guidance, see Preparation Steps, Step 4
2 Decide which pathway you are pursuing: training or experience (see checklist for details)
If training pathway, gather proof of training. Documentation must contain defined requirements, including lactation support, childbirth education, anatomy/pregnancy foundations, nonmedical comfort measures, and community resource development. Upload a syllabus if certificate lacks course hours.
If experience pathway, gather testimonial letters of support. You need three. At least one must be from a licensed provider, CBO, or enrolled doula. They must meet specific requirements. Download templates here.
3 Create a PAVE account
4 If you are a sole proprietor BUT organized as an LLC, corporation or partnership, you MUST enroll with a Type 2 NPI. This is the most common mistake that holds up applications. You will then also enroll yourself as an individual doula with a Type 1 NPI.
5 Upload all supporting documentation.
6 Track the status of your application through the portal. Respond quickly to follow-up inquiries from PAVE specialists.

TESTIMONIAL LETTER REQUIREMENTS
Three letters are needed. Download templates HERE.
You must have at least one letter from any of the following
- OPTION 1 -
FROM A COMMUNITY-BASED ORGANIZATION
Must be on the CBO's letterhead and signed by an authorized representative.
- OPTION 2 -
FROM A SPECIFIED LICENSED PROVIDER
Must be on the specified licensed provider's letterhead and include their NPI and provider type.
- OPTION 3 -
FROM AN ENROLLED DOULA
Must be on the doula's letterhead, include their NPI and address.
You must have at least two more letters from any of the following
IMPORTANT:Testimonial letters must contain NO protected health information or confidential patient information.
