TESTIMONIAL LETTER REQUIREMENTS
STEPS TO ENROLL AS A DOULA
- 1 Obtain your Type 1 NPI (individual provider NPI) - for application guidance, see Preparation Steps, Step 4
- 2 Secure a testimonial letter from either a CBO or licensed provider
- 3 Gather proof of doula training/certification
- 4 Obtain professional liability insurance
- 5 Create a PAVE account
- 6 Complete the individual provider application
- 7 Upload all supporting documentation
- 8 Submit application and track status through PAVE portal

PAVE Enrollment: Yes, must apply as an individual.
WHAT YOU'LL NEED TO APPLY
- Type 1 NPI (Individual NPI)
- Testimonial letter from either a CBO or licensed provider
- Documentation of doula training
- Business address( cannot be P.O. box)
- Professional liability insurance
TESTIMONIAL LETTER REQUIREMENTS
- OPTION 1 -
FROM A COMMUNITY-BASED ORGANIZATION
Must be on the CBO's letterhead and signed by an authorized representative.
- OPTION 2 -
FROM A LICENSED PROVIDER
Must be on the licensed provider's letterhead and include their NPI and provider type.
