BEFORE YOU APPLY
  • Define your services, populations and geography

  • Identify your payers and contracted managed care plans

  • ​​​​​​​Obtain the correct NPI type

  • Gather all required documents

  • Verify all documents are current

  • Confirm your business address is physical

  • ​​​​​​​Get Board approval for sharing and member information
 DURING APPLICATION 
  • Create your PAVE account using your NPI

  • Select the correct provider type

  • Complete all required fields

  • Upload all supporting documentation

  • Have an authorized representative sign

  • Review carefully before submitting
AFTER SUBMISSION
  • Track your application status through PAVE portal

  • Respond  promptly to deficiency requests

  • Check your email regularly for updates

  • Contact support if you have questions
REMEMBER ​​​​​​​The most common reasons for application delays or denials are: 
  • Using the wrong NPI type (Type 1 vs Type 2)

  • Incomplete documentation

  • Expired documents

  • Incorrect business address

  • Missing signatures from authorized representatives
