Community Health Worker (CHW) services are covered Medi-Cal benefits that help address social determinants of health.
WHAT ARE CHW SERVICES?
Community Health Workers provide culturally appropriate health education, outreach, navigation, and advocacy services to connect community members with healthcare and social services.
Note: Individuals CHWs do not enroll in PAVE. HOWEVER, a CBO must list at least one CHW on its application for it to be approved. You need a first and last name of a CHW and attest that that individual will complete the requirements that match certification
CBO REQUIREMENTS FOR CHW SERVICES
Must be a public or private non-profite with 501(c)(3) nonprofit status, OR
A fiscally sponsored entity of a 501(c)(3) nonprofit organization
Must provide 501(c)(3) nonprofit documentation
Must have at least one administrative location in California (cannot be a virtual office)
Must have signage, general liability and posted hours
Organizations must include the first and last name of at least one CHW with either a Certificate OR required work experience on the CBO application.
WHAT ARE THE CHW CERTIFICATION REQUIREMENTS FOR PAVE ENROLLMENT?
Per the State Plan requirements, if using the experience pathway, the CHW must obtain a certificate within 18 months of the first CHW visit. An organization/CHW without a qualifying experience pathway, MUST have the certificate at the time of the application. The certificate is never asked for in PAVE, but an auditor could ask for it in the future.
The physical location associated with your organization's operations, which can include where services are dispatched from or based. This CANNOT be a P.O. box or residential address.
